Biopsychosocial Determinants of Glycemic Control and Microvascular Health: An Interdisciplinary Approach

Резюме: Objective: To describe a theoretical model of the biopsychosocial determinants of diabetes self-management and microvascular health. Materials and methods: This is a theoretical study based on review and analysis of biomedi-cal and psychological literature on diabetes, its chronic effects and complications, and determinants of diabetes self-management and microvascular health. Results: The main psychosocial factors associated with diabetes are identified. A biopsychosocial model of diabetes self-management and microvascular health is proposed, including biomedical (type of diabetes, time since diagnosis, type and severity of complications, comorbidities), psychological (diabetes distress, depression, anxiety, type D personality, coping styles, self-efficacy), and social factors (social support, environmental stressors, socioeconomic status, access to medical service) as determinants of diabetes self-management (blood glucose monitoring, diet, exercise, weight management, adherence to medication, adherence to medical visits), that predict the peripheral microvascular activity (endothelial function and vasodilation). Discus-sion: Based on the proposed model, an empirical interdisciplinary study and a development of a prototype of non-invasive personalized home system for monitoring and predicting complications in the peripheral vascular system are planned. Conclusion: Interdisciplinary biomedical and psychological approach has the potential to integrate and address the most important diabetes-related determinants to support effective diabetes self-management and prevention and mitigation of complications, through the development of personalized and accessible digital solu-tions.
